Once you are registered for a seminar you need to fill out the forms in the bariatric patient information packet. If you prefer that we mail a packet to you, please call us at 501-227-9080. The packet includes all of the necessary forms and instructions. The completed information packet should be turned in at the seminar.
In addition to the patient information packet, you will need to submit the necessary medical documentation. Most insurance companies require documentation of your previous weight loss attempts, and many also require additional records which may include: a psychiatric evaluation, specific lab work, dietary counseling, or other testing prior to approval for surgery. You should contact your insurance carrier to determine if your insurance company has any specific requirements.
All patients will need a letter and documentation from their primary care physician stating previous weight loss attempts and outcomes of those attempts, as well as co-morbidities (other weight-related health problems). If you have previously had weight loss surgery additional testing and records will be required; please contact the office for more information. Every effort should be made to obtain all records prior to the seminar.
